Home health care brings medical care to your home. A doctor orders it, and our nurses and therapists carry out the plan — wound care, medications, injections, therapy and regular check-ups.

It is for people who need skilled medical care but not a hospital stay. Many patients come to us after surgery or when a condition like COPD, heart failure or diabetes flares up.

EliteCare Colorado is a Medicare-certified, CHAP-accredited agency, licensed as Class A and Class B (License #04Z659). Care can begin within 48 hours of receiving the referral and physician orders.

In-depth guide to home health care

Who qualifies for Medicare home health care

Medicare home health is for eligible patients who are homebound and need skilled care ordered by a physician. Leaving home must be difficult because of their health and require considerable effort or help. Being homebound does not mean they can never leave the house.

Help with cooking or bathing alone does not qualify someone for Medicare home health. The plan must include a covered skilled need, such as nursing or therapy. Our team reviews the referral and helps check coverage before care is scheduled.

Skilled nursing and wound care at home

Skilled nursing visits involve medical tasks that require a licensed nurse. An RN reviews the needs, and an RN or LPN provides ordered services such as wound care, injections or catheter care.

For an incision or wound, the nurse follows dressing orders and records healing. Family members can ask about care between visits. Changes are reported to the physician rather than changing treatment without an order.

Managing COPD, heart failure and diabetes at home

COPD, heart failure or diabetes may lead to a need for nursing after a health change or hospital stay. Visits can include ordered vital sign or blood sugar checks, medication review and education about the physician’s plan.

The nurse helps the patient and family understand their instructions and reports changes to the physician. Home health supports treatment but does not replace office visits, emergency care or the patient’s primary care relationship.

Why physician orders matter

Physician orders identify needed nursing and therapy services. They connect home care with the doctor’s treatment plan. Our nurses report progress to the physician so care reflects current needs.

A referral and physician orders are needed before ordered care can begin. Care can start within 48 hours of receiving both. If you are unsure whether orders were sent, tell us so we can review what is needed.
