• Information you give us in website forms: your name, phone number, email address, ZIP code, who needs care, the type of care you are asking about, and any message you write. Referral forms also collect the patient's name and the referring provider. Job applications collect your name, contact details, the role you want and a short message.
• Basic technical information collected automatically when you visit, such as pages viewed, the referring website, browser and device type, and approximate location based on IP address. We use this to understand how the site is used and to keep it secure.

Please do not include diagnoses, medications or other medical details in website forms. A member of our team will call you and collect what is needed in a secure way.

Our website chat is provided by LeadConnector (GoHighLevel); messages you send in chat are stored in our customer communication system.

We keep form submissions as long as needed to respond to you and for our business and legal records, then delete or de-identify them. If you become a client, your records are kept as required by federal and Colorado law.
