How we coordinate care with the referring physician
A registered nurse completes the first assessment in the patient's home and writes the care plan. For skilled nursing and therapy, the plan follows the physician's orders, and the nurse contacts the physician's office when orders need to change.
Therapy is provided by our own physical, occupational and speech therapists and by partner therapists, coordinated by our team. If a patient also needs non-medical help, such as personal care or homemaker services, the same agency can provide it under one care plan.
Payers we accept for referred patients
- Medicare (EliteCare is a Medicare-certified home health agency)
- Health First Colorado (Medicaid) and IHSS
- Blue Cross Blue Shield and Cigna (in-network)
- Private pay
